Elayne Forgie

Elayne has been a professional care manager for more than 29 years. She completed her professional coursework in health and wellness coaching at Wellcoaches School of Coaching, and in Advanced Clinical Dementia Practice, at the University of Michigan School of Social Work. 

She was a founding Board Member of the Florida Aging Life Care Association and is currently a member of the National Association of Social Workers, Case Management Society of America and the American Counseling Association. She continues to serve as the President and founder of ElderCare At Home and is the founder, President and CEO of the Alzheimer’s Care Resource Center.

Elayne is frequently acknowledged for her work on a local, state and national level as a caring and passionate elder care and Alzheimer’s advocate; professional trainer and educator, and for her life-long work on successfully meeting the needs of older adults, caregivers, and family members living with a diagnosis of Alzheimer’s disease, dementia or other cognitive impairment. She has an impeccable reputation and has helped thousands of families across the United States.


Elayne has served on the Executive Board for the Area Agency on Aging/Your Aging Resource Center and on the Advocacy Advisory Board for the Alzheimer’s Foundation of America. She is a former board member of the Alzheimer’s Association Southeast Florida Chapter.

She volunteers her time and resources and serves on both local and national advisory boards, where she continues to work with other recognized experts to improve the quality of care, and the quality of life, of those who love, and care for their aging parents and growing families.
